Application Process - Limited Licensure

Complete and submit the Licensure Application, supporting documentation and fees to PCS.

• Download and complete Licensure Application; click here for Licensure Application
• Download and complete the Criminal Offender Record Information(CORI) Acknowledgement Form; click here
• Board sends applicant Limited Licensure number to approved applicants.
• If application is deemed incomplete, Board will notify applicant of deficiency.
