IMPORTANT INFORMATION REGARDING CORI ACKNOWLEDGEMENT FORMS

Effective January 3, 2023, Board staff and resources moved to the Department of Public Health (“DPH”). Please note the following:

Beginning December 29, 2022, if you have not already submitted a CORI Acknowledgement Form with your application, you must submit a complete DPH CORI acknowledgment form available here.  Division of Occupational Licensure CORI Acknowledgement Forms are no longer accepted.

YOU MUST SUBMIT A COMPLETED APPLICATION AND CORI ACKNOWLEDGEMENT FORM TO PCS.

• ‍Massachusetts Physical Therapist
• ‍Massachusetts Physical Therapist Assistant
• ‍Download and complete the Criminal Offender Record Information (CORI) Acknowledgement Form

Allow three to four (3-4) weeks to process your application once PCS receives all documents.

To send your transcript/verification, please choose one of the two following options:

• Electronic Transcripts/ Verifications: PCS will accept final official electronic transcripts  received directly from the primary source (such as the School, Parchment, the National Student Clearinghouse, or State Licensing Board) via secure server to mbtranscripts@pcshq.com.
• Transcripts via Postal Mail: Official Final Transcripts and mailed veirfications must be mailed by the educational program or State licensing board  directly to PCS at:
