Eligibility Requirements

It is highly recommended that all applicants read the Board's Rules and Regulations.

Initial Licensure Requirements: (Applicants who have never been licensed before as an Optometrist)

  • be 18 years old
  • be of good moral character
  • have earned a college or university degree
  • have earned a degree from an accredited optometry program
  • have earned a passing score on the National Boards Parts I, II, III and TMOD
  • must complete a criminal offender record information request form

Licensure by Reciprocity Requirements: (Applicants who have been licensed before as an Optometrist outside of Massachusetts)

  • be 18 years old
  • be of good moral character
  • have earned a college or university degree
  • have earned a degree from an accredited optometry program
  • have earned a passing score on the National Boards Parts I, II, III and TMOD
  • ask the state(s) that the candidate is coming from if the state(s) would accept a Massachusetts candidate as a reciprocal candidate; the Board in question will need to submit this information in a letter to PCS--this is required for the file to be complete.
