Application Process

ALLOW THREE (3) TO FOUR (4) WEEKS TO PROCESS YOUR APPLICATION ONCE ALL DOCUMENTS ARE RECEIVED BY PCS

  • Applicant submits completed application, fees and required documents to PCS for an eligibility review
    • Download and complete Licensure Application; click here for Licensure Application
    • Download and complete Criminal Offender Record Information(CORI) Acknowledgement Form; click here

To send your transcript and/or verification Letter, please choose one of the two following options:

  • Electronic Transcripts and/or Verification Letter: PCS will accept final official electronic transcripts and verification letters received directly from the primary source (such as the School, Parchment, the National Student Clearinghouse, or the State Licensing Board) via secure server to mbtranscripts@pcshq.com.
    You may submit your application without filling out section G. (“certificate of pending graduation”) you may then have the dean of your optometry program fill out a separate copy of that section, electronically sign the page and send it, electronically, directly from the program to the following email address:

  • Transcripts and/or Verification Letter via Postal Mail: Official Final Transcripts and/or Verification Letter must be mailed by the educational program directly to PCS at:

Licensure by Reciprocity candidates:  Ask the state(s) you are coming from if the state(s) would accept a Massachusetts candidate as a reciprocal candidate; the Board in question will need to submit this information in a letter to PCS--this is required for the file to be complete.

  • PCS will notify applicant of deficiency or approval to take the jurisprudence examination
  • Eligible applicants take the jurisprudence examination at a PSI test center
  • Examinee receives unofficial score at exam site. PCS emails official score within 10 business days
  • Non-passing scores will have re-examination information enclosed
