IMPORTANT INFORMATION REGARDING CORI ACKNOWLEDGEMENT FORMS

Effective January 3, 2023, Board staff and resources moved to the Department of Public Health (“DPH”). Please note the following:

Beginning December 29, 2022, if you have not already submitted a CORI Acknowledgement Form with your application, you must submit a complete DPH CORI acknowledgment form available here.  Division of Occupational Licensure CORI Acknowledgement Forms are no longer accepted.

YOU MUST SUBMIT A COMPLETED APPLICATION AND CORI ACKNOWLEDGEMENT FORM TO PCS.

• ‍Massachusetts Occupational Therapists
• ‍Massachusetts Occupational Therapy Assistants
• ‍Download and complete the Criminal Offender Record Information (CORI) Acknowledgement Form

Please allow three to four (3-4) weeks to process your application once PCS has received all documents.

To send your transcript, please choose one of the two following options:

• Electronic Transcripts: PCS will accept final official electronic transcripts  received directly from the primary source (such as the School, Parchment, or the National Student Clearinghouse) via secure server to mbtranscripts@pcshq.com.
• Transcripts via Postal Mail: Official Final Transcripts must be mailed by the educational program directly to PCS at:
